Description of problem: `ceph-volume lvm activate` currently allows an ID and FSID, and this works great when coming from systemd or from within the 'create' process, but not so much for a user who was prepared a bunch of OSDs and now needs to figure out what the ID and FSID is.

Expected results: To be able to run these in addition to the currently supported ID and FSID:

    ceph-volume lvm activate /var/lib/ceph/osd/{cluster}-{id}
    ceph-volume lvm activate vg/lv

Additional info: The vg/lv should be for the data logical volume, and when using a path, it has to be a mounted path for prepared OSD that should contain the information ceph-volume needs
