Are you asking how to provision usNIC devices on Cisco servers?

If so, the usNIC deployment guide for Cisco UCS B series servers is probably your best bet.

Or, it might be easier to simply clone/duplicate the UCS service profile that is being used on cisco-b200m3-01 and apply it to cisco-b200m4-01.

I was unclear why cisco-b200m4-01 did not show the device info but cisco-b200m3-01 did.

From what I can ascertain, this isn't actually a bug, it was a configuration issue with the server itself. Please reopen if I've misinterpreted.
